The Tim Ferriss Show

How to Use Ketosis for Enhanced Mood, Cognition, and Brain Protection

The Tim Ferriss Show with Dr. Dominic D'Agostino 2026-01-07

Summary

Dr. Dominic D'Agostino provides a practical guide on using ketosis for mental performance and neuroprotection. Covers how ketones elevate GABA, quiet the mind, and support metabolic psychiatry. Tim shares how ketosis reversed his pseudo-dementia symptoms from Lyme disease in just 3-4 days.

Key Points

  • Ketosis benefits include quieting the mind and elevating GABA
  • Metabolic psychiatry uses ketones to address mental health conditions
  • Tim's Lyme disease-induced pseudo-dementia reversed in 3-4 days of ketosis
  • Spirochetes (Lyme bacteria) are glycolytic—ketosis may starve them
  • Practical protocols for achieving and maintaining ketosis
  • Long-term brain protection through metabolic flexibility

Key Moments

Ketosis quiets the brain: lowers glutamate, elevates GABA across seizure types

Ketosis silences seizures across all etiologies by lowering glutamate and elevating GABA.

"Obviously, it's silencing a broad array of seizures, independent of the etiology, temporal lobe seizures, you know, Lennox-Gasteaux syndrome, Gervais syndrome, rare forms of epilepsy. The ketogenic diet just quiets the brain, lowers glutamate, and elevates GABA. A brain-stabilizing, calming neurotransmitter. And, you know, that's why people gravitate towards alcohol, right? The GABA ergic effects of alcohol and benzodiazepines. So you can sort of, in a very gentle way, And that, I think, really echoes its broad application for metabolic psychiatry, which is everything from major depression to bipolar to schizophrenia to anxiety disorders to anorexia, nervosa. That's a very interesting and rapidly expanding application of ketosis that's being."

Ketosis stimulates adaptive immune response: relevant for Lyme, shingles, herpes

Beta-hydroxybutyrate in the blood stimulates the adaptive immune response, the body's targeted defense system.

"It's really incredible on a whole lot of levels. And I'm just lucky that I had the prior experience with the ketogenic diet to even have it occur to me as a last-ditch effort. And for people, if it wasn't clear, I mean, you explained it clearly, but glycolytic sensors."

Keto slows cancer but isn't a cure: 80% of cancers are highly glycolytic

Keto slows cancer growth in the 80% of cancers that are glycolytic and enhances response to standard therapies.

"People who've been on dozens of medications who go into remission after being treated on a ketogenic diet. It is astonishing."

Intermittent fasting made keto transition effortless: no fog, no low energy

Ferriss found that months of intermittent fasting beforehand made his keto transition the easiest ever -- zero fogginess, low energy, or irritability.

"But I was a little pissy, a little irritable for a little while, a little foggy. And then when I went into the keto-jank diet this time, it was the easiest transition I've ever had. I did not have any fogginess. I did not have low energy. I did not have what some people might call the kind of keto flu adaptive period."
Ketogenic Diet

Tim's keto-friendly cholesterol management: Repatha + ezetimibe for hyperabsorbers

Ferriss is a cholesterol hyperabsorber using Repatha and ezetimibe.

"But one to two seems to be the sweet spots for metabolic management of many different things and also just general metabolic health. And I think what you'd be doing, just doing low carb and periodic ketosis, is just enhancing your metabolic flexibility and maybe increasing the diversity of food that you're eating to prevent micronutrient deficiencies."

Prescription keto foods coming: gourmet meals hitting modified ketogenic macros

A company called MediFoods is creating gourmet prescription whole foods that hit modified ketogenic diet macros.

"Yes, okay, these benefits are. And now she's been on like Diary of a CEO podcast, I think, and some other podcasts. So I'd also like to mention I'm testing today even. There's a company coming out. I don't know if I can mention or not, but I will, Metafoods. And I've for years, I was kind of, there's no whole food-based ketogenic diet prescription foods. So I have no association with the company. I'm just testing their product, but they're called Medi Foods."
